Bent down and got acute attack - any advice

I have herniation to L4/5 and L5/S1 with arthritis in sacro illica joints - also herniation in neck disc and degeneration of other discs. I am 47 yr old female in UK.
I have been having problems for past year in that if i sit on a hard chair for longer than an hour then anything from 6-12 hrs later I will bend over even slightly and wham I get completely stuck with agonising pain in left side of spine at lowest point - assume this is either L4/5 or L5/S1
After this everytime i move in the slightest I get agonising pain and walking is excruitiating as you swing your leg or try and put your weight on it.
Usually within a day or two I am much better - I can tell from the initial attack and how painful it is as too how long it will last. Sometimes it takes a week before there is any improvement. Yesterday i spent 3 hrs on a hard chair at a birthday party meal and sure enough this morning I bent slightly forward and wham - worst pain I have had so far. I am in agony with every movemnent and every time I go from sitting to standing my whole lower back goes into spasm.
codeine and diazepan are not helping at all.
I have appt with chiropractor tomorrow morning but wondered if anyone else gets same problem and what is it - disc or facet joints??? I will ask chiro tomorrow but he tends to hedge his bets and say its both!

  • Welcome to SH :H I have the same problem and can't sit without pain at all. Perhaps you can try limiting the amount of time you sit because sitting exerts the most pressure on discs. If you have to sit, maybe a cushion can buy you some time. I herniated the same levels you did and understand how bad it hurts. Do you have nerve pain radiating down a leg? If so, I would be really cautious about seeing a chiropractor. I suggest you see a neurosurgeon or other spine specialist if possible. I understand how long it takes to get in with one in the UK. Using heat, ice, and stretching can help with this. Lying down to rest takes the most pressure off the injured discs, and so will reclining back on a comfortable chair with pillows. Maybe you can let your doctor know that the codeine doesn't help your pain much. Diazepam is very helpful with muscle spasms, and you can try taking an NSAID and a nerve pain medicine (Lyrica,Neurontin,Cymbalta) to help out. How long ago did you herniate those discs, and what treatment did your doctor recommend? :)
  • I have disc and facet joint problems also. I hope you're Dr. can refer you to a pain management Dr. as well as a MRI and Neurologist. Also let your Dr. know the pain meds aren't helping. If it's also your facet joints there are injections to help. I've had some injections that helped the pain alot. Take care and keep us posted on your progress. Charry

  • Thanks both for your replies much appreciated.

History is C5/6 herniation with osteophyte many years. Neurofibroma on thoracic spine with ddd. Moderate herniation to L4/5 L5/S1 on mri three months ago - previous lumbar mri year previously showed same herniation. Degeneration of sacro illiac joints.

I saw Neurosurgeon re neck disc but decided against surgery on his advice and am glad I did.

I have had no treatment of lumbar problems as am very frightened of invasive treatment but appreciate that if I am in this level of pain for long then i will gladly let them do anything to me!

I seem to be immune to codeine and tramadol they have no painkilling efeects on me at all and am reluctant to take gabapentin etc as I already have a number of the side effects they can give you hoho.

I have seen the chiro before for my thoracic problems with sucess so am hoping that he can help me again but I am also very wary of manipulation as well so won't let him do anyting too drastic.
  • I hope you're Dr helped you with your pain management and you reviewed your meds and their reaction on you. Have you had a pain management Dr? I've had 3 epis that helped and some facet joint injections. Good luck and be careful with th Chiropractor. I went to one and he wouldn't touch me so all I received was some TENS and excercises. Bye for now. Charry
