As some of you may know I have multiple Thoracic HNP (herniated disc). My worst at T4/5 and T6/7 with flares at T10/L1.... But by far the upstairs is worse than downstairs.

At my last NS appt about 3 weeks ago they said that I should consider a SCS (spinal cord stimulator) and sent the refferal to my PM office.

Yesterday I went in to see my PM for my monthly visit. Two of the PM docs in the clinic came in to see me. They don't want to do the SCS yet. They say it is a last option. They want me to go to another state (Utah or Arizona) and get a microdiscectomy.

My NS said they don't recomend the MD (say they hav a very low success rate and would probably have to go back in for the fusion any way). My NS said the SCS might buy me a couple years before I have to have the thoracic fusion.

My PM doc said the reason my NS doesn't want to see me get a Micro D is that he doesn't perform them and wont get paid....

Seems terribly confusing. I probably need to get another opinion I live in a remote city with a short supply of specialist..... I think there is one other clinic with 2 NS's and one clinic with 1 spinal OS (he doesn't do T-spine).

Any suggestions?

  • Should say worst T4-T8
  • I hear it's better to have the spine stabilized with surgery before a SCS so your PM Dr. is right on and it usually is used after a fusion.

Jim ( http://www.spine-health.com/user/jhowie)in this forum had surgery although after his SCS was put in though but it could have been revision TLIF surgery. I hope you can see another Surgeon to help you. Take care. Charry
