l4/l5 spinal fusion? To have surgery or not

Hi I herniated l4/l5 1 year ago. Have tried pt, cortisone injections with no relief. Saw the surgeon yesterday who told me after a 2nd Mri the herniation has not healed at all and now I have DDD and an anular tear. He said a discetomy is not an option as I have 90% back pain, my legs are pretty good still. He recommended a spinal fusion. I am concerned as I am only 36 with 2 small children. Told me I would be out of work for 4-6 weeks, 3 months of PT and 6 months before I feel better. Sounds like a rough recovery but I guess with the pain Im in now, it isnt that bad. Of course I am worried about having surgery and looking for some advice.
