LTD - Are you required to waive your medical privacy rights?

Hi all,

I'm applying for long term disability with with my employer's insurance company (metlife). There is a section titled "Authorization to Disclose Information About Me", which basically says:
1. I permit any doctor/health care provider to provide information to the insurance company (metlife), and
2. I permit Metlife to disclose any info to my employer

Am I required to sign this?

My guess is yes. And it's probably not a big deal either way for them to see my current and previous medical history. Heck, it may even help to show my terrible history. But, I thought it'd be worth looking into before signing my privacy away.


PS - I've been on STD for 3 months and applying for LTD for the first time now. If there are any previous posts or current advice you have for me please let me know.
