Back going out without reason, pain and shakey hands as well

I was told I had inflamitory in my back, neck, chest, hands and head when I was 15, I went through physiotherapy where I was told to crack the air bubbles out of my back, neck and hands and to take diclophenic and co-codimal.. I haven't taken any tablets since I was 18 ( they weren't doing anything) and when I moved to my new doctor she was confused about my condition, like she'd never heard of it.. I've managed the last few years, I'm now 25, but now my hands shake when I'm making my baby's bottles and other household chores, my fingers feel stiff, my back gives in when I'm not doing anything at all and hurts and makes me lose power in my legs for days.. Feel like I've lost control of by body.. I get very confused, replacing what I mean for another word, I put it down to being tired (8 month old not sleeping through the night) but its happening so often, morning, noon and night.. I don't want to make a fuss and go to the doctor, they'll tell me I'm tired and get me out ASAP, I know they will.. Anyone else have these symptoms? Any help is welcome..
