I just joined the forums and am hoping you all can give me your opinions quickly. Here is a little about myself. I am a 26 y.o. female with a long history of spinal problems including severe progressive scoliosis, herniated discs, sciatica, and 4 reconstructive spinal surgeries, one resulting in a T8 spinal cord injury.
I am currently on the fentanyl patch q48 hrs and Morphine IR 2-3 times a day. I am needing to have reconstructive bladder surgery in the near future (next 1-4 months, depending on results of testing on the 6th), and want to reduce my tolerance to opioids a bit prior to surgery. I see my PM doc later this afternoon, and want to discuss possible options with her.
Here is what I am considering, but want input/options/ideas/etc. from you all. I possibly want to reduce the strength of my patch by 25mcg/hr (still changing q48 hrs), and change from Morphine IR to Oxycodone IR or similar in a reduced strength but possibly more frequent so that I don't feel the effect of WD as I am tapering down, and decrease my dose/stretch out the timing as much as possible.
Do you all have any other ideas on breakthrough meds I might consider that are not as strong as Morphine? Of course I will be discussing all options with my treating PM doc and following what the doc recommends to a 'T'.
