upper right back pain?

I was shoveling dirt about 2 weeks ago. I was at work and we had about 1500ft. to shovel. We were shoveling loose dirt but I got over zealous and tried shoveling as fast and hard as possible. I was basically in a 90 degree angle for 5 minute stints over a 45 minute period. I had about 3 times the amount of dirt the shovel could normally hold. Well the next day my back hurt pretty bad well after a week it went away. I woke up yesterday in serious back pain the same spot. It hurts pretty much always it was really hard to sleep. When I cough or sneeze its like a shot of pain. Inhaling is unpleasant as well.

What is this and how can I make better?

Is it something I always have to live with?

Do I need to see the doctor?
