Still In Pain!!!

Ok guys, so long story short I had a laminectomy and microdiscectomy done about 6 months ago to the day (levels L3-L5). I was on a pretty good pace recovering, feeling better and better slowly but surely. I knew that I would never be 100% again going into the surgery and I wasn't necessarily expecting that, I just wanted enough relief to get me through the day without much (if any) pain. I completed my physical therapy and continued to do my at home exercises bi-daily. Fast forward to last week...I wake up one morning and BOOM it felt exactly like it did prior to surgery. Most of you know the crippling feeling of not being able to even roll out of bed. I'm going to see my surgeon tomorrow but I wanted to post this first.

Ok my two questions:
1. What questions should I ask the doctor?
2. Do you guys think that surgery is going to be necessary again?
