Back pain for years

Hello everyone, I am very new on this forum and I have joined hoping to get help in understanding my conditions.
I am an active person and did all sorts of sports over the years, martial arts, weights lifting etc. I am 47 now and my back claims the price for all these activities but if I'm being honest, I have experienced back pain all these years at some point.
It was pretty much an on/off pain period. A bearable pain though. However, I decided to have it investigated, went to my GP and got sent to X ray. The results are as follows: There is anterior wedging of L1 which appears to be chronic in nature. There is mildly reduced disc height at L4-5 and L5/S1. Anterior osteophytes seen at all levels, more marked at L2-3. There is mild posterior slip of L2 on L3.
I am really worried and any help, advice etc will be greatly appreciated.
I just want to understand what all this mean and what I can do to treat it. My GP recommended physiotherapy.
