What surgical options do I have?

I have a few spinal problems that cause me to be in severe pain 24/7. I have spondylosis (completely missing the pars bone), spondylolisthesis (grade 1), moderate osteoarthritis in the vertebra of my lumbar region and moderate foraminal narrowing on both sides. All of these issues are in the L4-L5 region. I have tried many medications and an extensive physiotherapy program, neither of which have worked. Last week I had steroid injections into my lumbar region to try and reduce my severe pain (my consultant wanted to try all conservative methods first) and I felt a small amount of relief for about 3/4 days, but now my pain is just as bad as it was, so unfortunately I'm back to square one. I'm very fortunate to have a very understanding and highly recommended spinal surgeon as my consultant, and he knows how much pain I'm in and how much it affects my life. He suggested that I try the steroid injections, but said that if that doesn't work then we would need to start discussing surgery. I don't have my next appointment until about 6 weeks from now, so I would really appreciate some information on what types of surgery I could have or just any previous experiences with low back pain and surgery, as he didn't elaborate the last time I spoke to him.
