Desperately seeking some advice.......

Hi All. I am a 37y/o active female, last September I had microdiscectomy surgery at L5/S1 for a disk herniation. Surgery was successful and for 5 glorious weeks I was completely pain free.
6 weeks out, I started to develop pain, which I attributed to 'over doing it' but no amount of rest helped. Fast forward to spring and I had 5 sets of facet block injections (starting at L4/L5, three at L5/S1 and one at SI joints). Injections at L5 and SI showed improvement in pain for as little as 4 days and as long as 9. This August, surgeon performed a facet rhizotomy at L5/S1 and there seemed to be improvement for about 2 weeks and now pain is back in full force.
Surgeon said a spinal fusion may have to be done as a last resort. But I am now not convinced it would work. After all that has been done, could it be possible that there is another area affected??? SI joints, tailbone, pelvic structure??? I don't want to out myself through a difficult surgery with a long recovery to still be in pain from something else.
Talking to surgeon next week..... Been to chiropractor...... Been back to physical therapist........
I am an avid equestrian and need to be comfortable to train and compete. And when the pain is so debilitating that I can barely stand when I roll out of bed, I just don't know where to go from here. Even though about paying to have a private MRI done if I thought that would help.
Pain meds (nucynta) help, but I hate taking them and don't want to be a lifer.........
At my wits end....... Help!
