Extreme lower back pain

I'm 19 years old and started feeling pain in my back many years ago. It's a piercing pain just to the right, and sometimes left of the spine right above my hips. The first time it happened I stepped down from a ledge and it occurred. I live on a dairy farm so usually the pain reoccurs while I am working on the farm. The pain is usually so intense that I can hardly function however it usually disappears after a few days. Today it happened again, except I was bending down to dry myself off with a towel and now I can hardly walk. Ibuprofen doesn't seem to help and ice only helps slightly. Is this something that I should be worried about or is it just a weak muscle? I havn't seen a doctor yet for my back pain and I only take ibuprofen for the pain. Any suggestions would be greatly appreciated as this pain has been happening on and off again for years and I'm really annoyed by it. Thanks!

Edit: In response to the welcome message, I can't really offer information about doctor visits as I havn't been able to visit one. I only take ibuprofen or advice type medicine for pain and ice.
