How to tell if my doctor did my surgery

I had hernia surgery and also a hydrocele surgery. In the office doctor said he was doing both at same time. Said he would cut my sac open for the hydrocele surgery. After the surgery the was no incision on my sac. I asked his about it and he said he did the repair through my opening for my hernia repair. My sac remained the same size after the surgery. I think he forgot to do the surgery. How do I get the doctors notes to prove this. I have been in pain for two years after my surgery.

No one on this site can answer those type of questions. You need to call the hospital and get the surgical notes
