When is surgery a necessary thing?

I just was told I have two bad bulging discs in my thoracic spine T9-8, T8-7 - the pain is so bad that if I'm in tears half the time and after my lumbar fusion; I have a high tolerance for pain. It also doesn't help that due to prior authorization my extended release medicine (zohydro at the moment, they filled the first one on the savings card and denied the newer dose yay) and I went through a week of withdrawal because there was no way to taper the dose - an increase in pain there. But that's over now (I think) and I'm still in a lot of pain - about a 6/7 most of the day. A heating pad helps ease it but I can't sit all day, it's driving me insane and yes, I use those thermacare ones from time to time they are just cost prohibitive .

I'm going through PT at the moment and I ended up crying on the floor because of pain and then having to cancel my second session that week due to pain. I haven't tried ESI or facet injections or nerve burns in that area yet but none of those things were beneficial to my lumbar region in the end.

I don't know much about the thoracic spine at all though i am reading about it but I guess my question is - does surgery help the area and pain? It helped my lower back and no, I don't want to just have surgery - I just can't stand the pain anymore because it's interfering with my life all the time. I know there are people in worse shape than me but we all have our breaking point. Thanks all.
