Approval for fusion

Has anyone had trouble getting approval for a fusion due to the fact that their problems are related to herniated discs and not strictly DDD or a deformity? I have herniated both L5-S1 and L4-L5 2 times and as a result have reduced disc height, but the need for surgery is based on the fact that I keep reherniating. I have been reading a lot of insurance requirements and it appears many don't like to approve fusions for the treatment of disc herniations. Has anyone had a fusion because of this and did they have trouble getting it covered?
