Back Pain only at night

Please help me if you have any ideas as to what this might be.

Sporadically, I have a serious bout of severe pain that seems to be starting in my lower back. I have the most severe pain in my hips (pelvis), my knees, and sometimes my ankles. The pain doesn't run the length of my legs.....it only hurts IN my hips, knees, and ankles, as if it skips by thighs and calves. It only happens at the end of the day, and once I go to bed and actually fall asleep, I am fine in the morning.

This pain has never happened at any other time of day.

This happened last night, and I was in such severe pain that I was pacing and at times crying (I have a high pain tolerance from bouts of kidney stones in the past, so I know something is wrong).

It only happens about once a month or so, and sometimes I have periods of a few months without any of these episodes. They truly do feel like "episodes" and I can't figure out what triggers them or what is wrong. I see a chiropractor once a month, but unfortunately since I see him during the day, he has never seen me IN this condition, and only does minor adjustments when I'm there.

Help me please, if you know or have experienced this too.

One of the most important things that members can do is to provide the rest of the community with as much information about themselves as possible. It is so very difficult for anyone to respond when we do not have enough information to go on. So many times we read about members who have different tests and they all come back negative. The more clues and information you provide, the better chances in finding out what is wrong, The fact that your test results are negative does not mean that you are fine and without any concerns. Many times it takes several diagnostic tests and procedures to isolate a specific condition.

Here are some questions that you should answer:

  • - When did this first start?

    . Year, Your age, etc
- Was it the result of an accident or trauma?
- Are there others in your family with similar medication conditions?
- What doctors have you seen? (Orthopedic, Neurosurgeon, Spine Specialist, etc)

  • . Which doctor did you start with? Ie Primary Care Physician
    . Who are you currently seeing?
- What Conservative treatments have you had? Which ones?

  • . Physical Therapy
    . Ultrasound / Tens unit
    . Spinal Injections
    . Acupuncture
    . Massage Therapy
- What diagnostic tests have you had? And their results (MRI, CTScan, XRay, EMG, etc)

  • . Summarize the results, please do not post all details, we cannot analyze them
    . How many different tests have you had over the years? Similar results?
- What medications are you currently using? (details, dosage, frequency, etc)

  • . Name of Medication
    . How long have you been using this?
    . Results
- Has surgery been discussed as an option? (If so, what kind)
- Is there any nerve pain/damage associated?
- What is your doctor’s action plan for treating you?

Providing answers to questions like this will give the member community here a better understanding
of your situation and make it easier to respond.

Diagnosing spinal problems can be very difficult. In many ways it’s like a game of clue. Especially, when the diagnostic tests come back negative – no trouble found! Then it’s up to the patient and the doctor to start digging deeper. The doctor is like a detective. They need clues to help them move along. So, you as the patient need to provide the doctor with all sorts of clues. That is like it is here. Without having information about a condition, its impossible for anyone here to try to help.

    Set up an appt. with your doctor. You may want to get a full blood panel if you have not had an annual physical exam in a long time.

    In the meantime, here are some questions:

    1. What is your mattress like?
    2. Does your job's daytime activity have anything to do with nighttime pain?
    3. Do you typically sleep on your side, back, stomach, or a combo of those?
    4. Have you tried sleeping other ways?
    5. Are you under a lot of stress?
    6. What kind of shoes do you wear? Do they provide enough support?
    7. Are you overweight?
    8. Do you smoke? (Smoking has been linked to higher risk for chronic pain)
    9. What is your Vitamin D level? (Also linked to chronic pain)

    Knowing your history of back pain would help too (any previous injuries, etc.).

  • Here are my answers

    1. What is your mattress like?
    My mattress is very comfortable in my opinion. It's not a spring mattress though. It has some kind of filling and is firm (more like a firm cushion). I don't wake up in pain, but I guess it still could be the mattress.

    2. Does your job's daytime activity have anything to do with nighttime pain?
    It's possible. I am a teacher, but I teach in the evening. So I am teaching from about 5-9pm, 4 days a week. It's possibly connected with standing from teaching.

    3. Do you typically sleep on your side, back, stomach, or a combo of those?
    I have a combo, but I usually end up on my stomach.

    4. Have you tried sleeping other ways?
    Yes, the back pain is sporadic and doesn't seem to be affected by the way I sleep ( or so I think).
    5. Are you under a lot of stress?
    My stress is moderate but not high. I am a teacher so I am busy.

    6. What kind of shoes do you wear? Do they provide enough support?
    I wear supportive shoes. In fact I have just bought orthopedic supports for the inside of my shoes (about 3 months ago) and my other shoes are already supportive. But I don't wear shoes at home, and sometimes my feet hurt from that.

    7. Are you overweight?
    Yes, but I am in the process of losing weight. I have hypothyroidism which makes it a particularly difficult battle.

    8. Do you smoke? (Smoking has been linked to higher risk for chronic pain)
    No, never have.

    9. What is your Vitamin D level? (Also linked to chronic pain)
    I take 3,000 IU a day (as prescribed by my doctor) because I had a deficiency.

    Knowing your history of back pain would help too (any previous injuries, etc.).

    I have never had injuries, just sporadic muscle tension and aches and pains. There was a time in college where my back would "catch" every time I bend over, but that went away on it's own. I see a chiropractor/physiotherapist, and he corrects lumbar and thoracic areas, sometimes my hips are out of line.
