The spine condition called isthmic spondylolisthesis occurs when one vertebral body slips forward on another because of a small fracture in a piece of bone that connects the two joints on the back side (the pars interarticularis) of the spinal segment.

Right leg pain while walking.

History of present Illness

Patient is a 43 year old physician that has had years of on again off again leg pain. The current episode of pain started over one year ago. He is otherwise healthy and previously very active. He has no pain with sitting, but if he tries to walk more than a 100 feet pain will radiate from his back down his right leg and into the big toe. If he sits down the pain will resolve. Although he can no longer jog, he can ride a stationary bike without any problems.

Spondylolysis is a condition in which the there is a defect in a portion of the spine called the pars interarticularis (a small segment of bone joining the facet joints in the back of the spine). With the condition of spondylolisthesis, the pars interarticularis defect can be on one side of the spine only (unilateral) or both sides (bilateral).

Isthmic Spondylolisthesis: Chiropractor Viewpoint

Chief complaint

Intermittent right lower extremity radiating pain exacerbated through walking 100 feet, standing extension and relieved with sitting/forward flexion.

Exam and imaging studies

Mild weakness in extending the right first digit. No signs of neural tension during SLR, and hip mobility is adequate. Plain film radiography and MRI scans show isthmic spondylolisthesis at L5-S1. Early degenerative disc disease with disc bulge and right foraminal stenosis at L4-L5 impinging the L5 nerve root.

Isthmic Spondylolisthesis: Surgeon Viewpoint

This patient presents a long-standing isthmic spondylolithesis. His pain is well described as occurring in the L5 nerve root-pain and weakness in his big toe. An important part of his history is the extent to which this limits his activities. It is unreasonable for a healthy 43-year-old to be only able to walk 100 feet, when there are means to treat the underlying problem.

As in most instances, one can either entertain non-operative or surgical options. The nonsurgical treatments however are less likely to have long term benefit.

Profile and Diagnosis of Spondylolysis

Spondylolysis develops most commonly in adolescents, most typically in 10 to 15 year olds. The majority of adolescents with spondylolysis do not have symptoms, or their symptoms are mild and are often overlooked. If the spondylolysis is not correctly identified and managed, there is a chance that the affected area may heal incorrectly, resulting in the possibility of continued stress that can lead to the slippage of spondylolisthesis and recurrent low back pain.

Spondylolysis Treatments

Clinical studies have found varying degrees of healing using conservative (non-surgical) treatments (such as bracing) ranging from 73% healing of early-stage spondylolisthesis versus 38% healing in those with progressive disease. One study of adolescent athletes found that 37% of them showed signs of healing at the pars defects after 2 to 6 months of bracing.1

Isthmic Spondylolisthesis: Physical Therapist Viewpoint

This patient's subjective history and physical exam both point to the fact that placing his lower lumbar spine in a relatively forward flexed position tends to relieve symptoms. Although we know he has a disc bulge present at L4-L5 on MRI, we have found symptoms to be more consistent with his spondylolisthesis at L5-S1 and will treat accordingly.

Degenerative Spondylolisthesis

Every level of the spine is composed of a disc in the front and paired facet joints in the back. The disc acts as a shock absorber in between the vertebrae, whereas the paired facet joints restrain motion. They allow the spine to bend forwards (flexion) and backwards (extension) but do not allow for a lot of rotation.
