History of Cold Laser Therapy

Cold Laser Therapy has been used in clinical practice all around the world for over four decades. In 1916, Albert Einstein conceived the theory of Light Amplification through Stimulated Emission of Radiation or LASER. In 1967, Professor Andre Mester began using low power lasers in medicine. Dr. Mester is recognized by many as the grandfather of laser therapy.

F.D.A Recognition of Cold Laser Therapy

The F.D.A classifies medical lasers into three categories:

Rechargeable Spinal Cord Stimulators for Chronic Pain (Research Article)

Back pain relief – overview of neurostimulators

Spinal cord stimulators are battery-powered electronic devices enclosed in a small metal container that are surgically implanted under the patient’s skin in the low back area. They send low levels of electrical energy to nerve fibers with the goal of preventing pain signals from reaching the brain. Depending on the patient, the pain is to some degree replaced with a feeling that is often described as a tingling or warm sensation.

Treatment by a Doctor of Osteopathy

After a thorough examination and diagnosis, treatment may include osteopathic manipulative treatment (OMT), medications, various types of injections and/or back surgery.

The number of OMT treatments required varies with each patient and condition. While there exist no “hard and fast” rules as to the duration and exact number of treatments required, objective improvement in pain and/or functional abilities should become apparent in as few as five to six consecutive treatments, unless there are prominent complicating factors.

(Research Article)

Percutaneous neuromodulation therapy (PNT™) is a new, non-surgical treatment that is based on the premise that electrical stimulation applied to the deep tissues in the back can help provide relief for certain types of lower back pain.

The treatment has recently received FDA clearance for treatment of lower back pain. It is minimally invasive and performed in the doctor’s office.

Chronic Pain Management - Patients' Advice: Part III

Additional patient advice on chronic pain management

Pilates and exercise keeps chronic pain manageable

I had back-to-back surgeries in June of 2003 to remove pieces of L5-S1. After the 2nd surgery, I still had considerable pain in my left calf and foot. I did physical therapy, had injections in my back, see an acupuncturist everymonth and do Pilates twice a week. I still have the pain but it is manageable. It only gets worse when I fail to keep up with my exercises.

Perspectives on Pain Management

Dear Spine-Health Member,

For many people, managing chronic pain is a constant battle. In addition to dealing with the pain, it's hard to know which treatments to rely on, such as how much to depend on pain medication versus other options, when to turn to spine surgery, which type of spine specialist to see, etc.

Therapeutic Nerve Blocks for Neuropathy

Purpose of a nerve block injection for neuropathic pain (neuropathy)

A nerve block is an injection of an anesthetic directly in the area of the affected nerve. The purpose of a nerve block in treating neuropathic pain is to interrupt transmission of the pain signal to the brain. If the pain signal does not get to the brain, then pain is not actually felt or perceived by the patient.

Injections in the following locations can sometimes produce short-term pain relief: trigger points, peripheral nerves, plexi, dorsal roots, and sympathetic nervous system.

In many ways a chronic pain management program for a potential spine surgery candidate is similar to that for any other chronic pain condition. The program is multidisciplinary in nature, aimed at improving function, reducing or eliminating use of painkillers, providing emotional stabilization, and teaching cognitive behavioral techniques for coping with pain as well as reducing maladaptive thinking.

Interferential Current (IFC)

Interferential current is essentially a deeper form of TENS. In essence, IFC modulates a high frequency (4000 Hz) carrier waveform with the same signal produced by a TENS unit. The high frequency carrier waveform penetrates the skin more deeply than a regular TENS unit, with less user discomfort for a given level of stimulation. Deep in the tissues, the carrier waveform is cancelled out, resulting in a TENS-like signal deep under the skin.

If your back pain or neck pain has persisted despite a variety of treatments - and maybe even after spine surgery - you may understandably feel frustrated and miserable. Pain can be very cruel - making it hard for you to undertake or enjoy even the simplest daily activities.

Dear Spine-Health Member,

When you’re dealing with chronic or ongoing episodes of back pain/neck pain, sometimes it seems that half the battle is just finding the right kind of spine specialist to diagnose and treat your pain.

Should you go to a chiropractor or a medical doctor?

Transcutaneous electrical nerve stimulation, or TENS unit (also called electrotherapy), involves placing an external device over the skin that applies low-voltage electricity via electrodes. Generally, TENS units are applied for moderate to moderately severe acute to subacute pain, and for some people they may be effective for chronic pain as well.

Electrotherapy

What is electrotherapy?

Electricity has been used to treat pain for over 100 years. Early proponents of electricity were labeled as charlatans, but recent scientific studies have proven that electricity can reduce both acute and chronic pain.

The exact mechanism of electrical stimulation’s beneficial effect remains controversial. Electrical stimulation may directly block transmission of pain signals along nerves. In addition, electrical stimulation has been shown to promote the release of endorphins, which are natural painkillers produced by the body.

Advanced treatments for neuropathic pain (neuropathy)

When oral medications and/or nerve blocks do not sufficiently control the pain, advanced pain therapies or implantable systems may be effective for treating neuropathy. These systems are designed to interrupt transmission of pain signals from the spinal cord to the brain. If the neuropathic pain signals do not reach the brain, then the patient does not actually feel the pain.

The Integrated Approach to Back Pain Treatment

At Spine-health.com, we are often asked which physician is the most appropriate to treat an episode of back pain. Our answer is usually "it depends". There are many different types of health professionals who treat back pain, and each has different training and interests.

First, it is important to note that no one type of physician treats all types of back pain, neck pain or other musculoskeletal conditions. For example:

Galvanic Stimulation (GS)

Galvanic stimulation is most useful in acute injuries associated with major tissue trauma with bleeding or swelling. In contrast to TENS and IFC units, which apply alternating current, galvanic stimulators apply direct current.

Direct current creates an electrical field over the treated area that, theoretically, changes blood flow. The positive pad behaves like ice, causing reduced circulation to the area under the pad and reduction in swelling. The negative pad behaves like heat, causing increased circulation, reportedly speeding healing.

Treatment Options for Neuropathic Pain

Treating pain from neuropathy

Currently there is no proven treatment to prevent or cure neuropathic pain (neuropathy or nerve pain). Instead, the primary goals of treatment are to reduce the pain as much as possible, balance the negative side effects of the treatment, and help patients manage any unresolved pain.

It is thought that early identification and treatment of intractable, chronic pain can help in effectively treating the pain.

Interdisciplinary Chronic Pain Management vs. Back Surgery: Which is Right for You?

Chronic pain management

Patients who suffer from chronic back pain understandably want to find a solution – and many hope for a ‘silver bullet’ that relieves most if not all of their pain. For many patients, the hope is that surgery is the best answer to relieve their pain quickly.

The patient may use a TENS unit at home for back pain relief on a long-term basis. TENS units are about the size of a pack of cigarettes and typically cost $250 - $700. All units allow the user to adjust the intensity of the stimulation; some units also allow the user to select high-frequency stimulation (60 - 200 Hz) or low-frequency stimulation (<10 Hz).

History and Philosophy of Osteopathy

History of osteopathy

In the late 1800’s a doctor named Andrew Taylor Still developed the field of osteopathic medicine and is considered its founder. He was a pioneering doctor in the study of how the attributes of good health could help doctors understand disease and illness.

Through his experience with patients and research, he decided there was a better way to treat patients than the medical practices of the time. Because his new ideas were not accepted in the medical community, he established a new philosophy of medicine in 1874 called “Osteopathy.”

Getting Better Care for Your Back Pain

One challenge with treating back pain is that there are not that many absolute rules. For example, a spinal condition that appears relatively minor can lead to severe back pain, but a very serious condition can be barely painful at all. Similarly, some patients with a spinal condition will develop chronic pain, while others with the same condition will not.

Neuropathic Pain Management and Medications

Medications may be first treatment option

Given the intense nature of neuropathic pain, medications are frequently a first line of treatment. However, finding an effective medication takes a trial and error approach and can at times be frustrating to both the patient and the doctor. In addition, the side effects of many medications can be problematic.

Integrated Spine Clinics for Back Pain Treatment

An integrated spine clinic brings together many of the different spine specialists to work collaboratively in one clinic. For example, it is becoming more common for orthopedic spine surgeons to work with neurosurgeons, invasive physiatrists to work with anesthesiologists and neuroradiologists in a pain clinic, and chiropractors to work with physical therapists.

Benefits of integrated spine clinics for back pain

Ideally, the integration of spine care affords several benefits for the patient and back pain treatment:

Pain Management Specialists

Health professionals in many fields have completed specialized training in pain management. Currently, there is no single field of medicine or health care that represents the preferred approach to pain management. Indeed, the premise of pain management is that a highly multidisciplinary approach is essential.

Pain management specialists are most commonly found in the following disciplines:

Pain Management After Microdiscectomy Surgery

Immediate post-operative pain can be managed with a combination of non-steroidal anti-inflammatory drugs (NSAID’s) and a mild pain pill (e.g. Darvocet, Vicodin). The narcotic pain medications (Vicodin) are addictive and should only be used for a short period of time (less than two weeks).

As the discomfort from back surgery subsides (usually about 1 to 2 weeks) the patient can move towards substituting Tylenol for the narcotic pain medications.
