1/07- Went to ER, MRI, Diagnosed with severe L5-S1 herniation and lumbar stenosis (after dealing with back and leg numbness for over 5 years)

3/07- Had first Transforaminal ESI, did not relieve any pain

6/07- Laminectomy and Discectomy at L5-S1

8/07- Symptoms start to reappear

9/07- MRI of cervical shows 3 bulging discs at C4-C5, C5-C6, C6-C7

11/07-MRI of thoracic shows herniation at T8-T9 and congenital fusion at T4-T5 with rudimentary disc space

7/08- MRI of lumbar shows osteoarthritis in L3-L4, L4-L5 and scar tissue encasing s1 nerve root on the right side

8/08- Had Facet joint injection now having symptoms in my left leg, CT shows L5-S1 reherniation on left side

Had another Facet joint injection 3 weeks later, still no relief.

10/08- Facet nerve block was unsuccessful and I began to realize that local anesthetics like lidocaine do not numb me. Therefore, all injections have been very painful for me, even though they shouldn't have been.

Currently seeing: Neuro, PT, PM
Meds: Lyrica 50mg/day, Soma, Percocet
Diagnosis: DDD, Lumbar Stenosis, Osteoarthritis

21 years old.
