Two previous lumber surgeries, spinal cord stimulator (wire is not connected to spine, rendering it useless). Multiple injections/blocks.
L1-2, disc bulging w/venrtral flattening of the thecal sac. Facet arthropathy.
L2-3, Facet arthropathy
L3-4, Left paracentral and left foraminal disc protrusion w/ mild indentation of the thecal sac and narrowing of the left L3 neural foramen w/ discogenic disease abutting the nerve root in the foramen. Facet arthropathy
L4-5, Disc protrusion w/ventral indentation of the thecal sac. Facet arthropathy. DD, facet arthropathy and ligamentum flavum hypertophy is distorting of the shape of the thecal sac, central stenosis in the spinal canal. Narrowing of both L4 neural foramina.
L5-S1, Disc interspace is substantially narrowed. Facet arthropathy. Disc margin spurring is present. Marked narrowing of the L5 neural foramina, Nerve root impingement.
Meds: Kadian 80mg 2xday, Zaniaflex 4 mg 3xday, Gabapentin 800 mg 1xday, Effexor XR 75 mg 1xday
