Diagnosis: Right L5 radiculopathy secondary to recurrent L4/L5 disc herination.
Operation: Hemilaminotomy at L4, partial facetectomy and removal of herniated and extrended disc L4/L5.
1989:
Diagnosis: Right S1 radiculopathy secondary to herniated disc L5/S1
Operation: Hemilaminotomy, partial facetectomy and foraminotomy of S1 root and removal of herniated disc
1994:
Diagnosis: Left C5/C6 radiculpathy secondary the disk herniation C4/C5 and cervical spondylosis and osetophyte C4/C5, C5/C6.
Operation: Anterior cervical diskectomy, removal of osteophytes and two Level anterior iliac bone graft fusuion C4/5 and C5/6

Diagnosis: Cervical radiculopathy and chronic axial neck pain secondary To cervical spondylosis, disc osteophyte, foraminal stenosis Right C6/C7, status post 2-level anterior fusion in the past

Diagnosis: Herination at T1/T2 Discs
2006:
MRI. L4/L5 right paracentral protrusion and oxtrusion contacting the Right L5 nerve root. Grade 1 posterior spondyloisthosis of L4 on L5 with severe narrowing
Of the intervening disc space. Desiccation of the L5/S1 interverterbral disc space and congential Narrowing of the spinal cord. There is a reversal of lumbar curvature.
L5/S1 mild foraminal stenosis with potential root contracts and right Protrusion.
MRI Cervical
Marked degenerative narrowing of the T1/T2 disc space with endplate Osteophyte formation.
