- Injured by being blown off military aircraft, thrown off horses, motorcycle accident and stood up wrong with 120lbs of wet gear on my back.
- PT and OM during 80's and 90's
- Began suffering migraines and electric shock headaches in 1998
- Diagnosed with Occipital Neuralgia
- Treated with anti-seizure meds and PT
- 2002 menigitis
- July 9 2003 blew out L5-S1 with wet gear
- Between Jan 2004 and June 2004 underwent 5 surgeries on L4,L5,S1 (right side)
- July 2004 ICU with Lumbar shunt and revision
- July 20 2004 started 9 weeks open wound healing on pocket created by CSF leak
- Dec 25 2004 reinjured lower back
- April 1 2005 decompression L4,L5,S1 (left side)
- April 2 2005 L5 fractured while in hospital, facet capsules blown
- April 6 2005 Total laminectomy L4 and L5. Discovery of CSF leak source, large dural patch
- May 10 2005 Rhizotomy C2-C3
- August 2006 Surgery right knee meniscus tear
- June 21 2007 C2 Ganglinonectomy attempt #1. Profuse bleeding episode halted surgery and emergency angiogram of vertebral artery performed.
- June 22 2007 Ultra Rapid Opioid Detox
- June 28 2007 C2 Ganglionectomy completed with C1 laminotomy
- July 10 2007 onset of Nystagmus, vertigo and hearing loss due to vasular injury to 8th cranial nerve
- Jan 2008 confirmed Muscle atrophy left side of neck, Winging scapulas
- C4-C5, C6-C7 bulging discs
- Feb 2008 five rhizotomies lesser and greater occipital nerves
- May 19 2008 start C-spine SCS trial
- May 22 2008 Permanent implant C-spine SCS
