2004: Cervical - Multilevel DDD w/o evidence of significant stenosis or neural compromise.
2004: Fibromyalgia, disabling.
2007: Cervical - C5-6, Moderate left foraminal stenosis and mild foraminal stenosis; Mild degree of straightening of cervical lordosis as well as reversal of cervical lordosis. C4-5, Mild loss of disc height and disc signal. C6-7, Mild loss of intervertebral disc height & mild posterior disc protrusion w/o significant foraminal or canal stenosis.
Diminished left bicep reflex, MS ruled out.
2007: Lumbar - L5-S1, Sacroilic joints misaligned & displaced resulting in facet arthropathy bilaterally; disc protrusion w/loss of intervertebral disc height; small amt of posterior superior disc space hyperintensity. L4-L5 & L5-S1, Some loss of synovial fluid at right facet joint resulting in facet arthropathy and mild ligamentum flavum infolding; moderate left lateral recess stenosis & moderate left foraminal stenosis. Disc bulge may contact the exiting L5 nerve root.
