FEMALE. Former tennis player with a bad ball toss on my serve. 6 ESI; 9+ facet injections; 2 rhizotomies; SURGERY 12/19/2006--Laminectomy and fusion L5-S1. Unsuccessful. Radiculopathy from damaged L5 nerve. Ow! Spinal cord stimulatory 4/2009 provides considerable relief. A-h-h Kadian 30mgs 2Xday plus 4mg. Dilaudid if ever needed.
