Headaches, Thoracic herniated disc, trapezius spasms

I have been dealing with headaches for years now, as well as right trapezius tension. The pain has recently gotten so bad that I went to an orthopedic surgeon followed by a pain management specialist. The doc found that I have a herniated disc between T7/T8. I have received a first round of cervical facet injection to relieve headache pain (which worked for about 6 weeks) and just recently an epidural in my lumbar to relieve the pain in my trapezius.
I experienced severe lower back pain immediately after the injection, followed by flushing of the face and elevated body temperature for the past week. For the past 2 days I have had the feeling of a pulled muscle in my right trapezius (exactly where I get pain).
I need to get to the bottom of these medical issues. I have a 20 month old daughter and am a professional in the field of education. I cannot keep up with this pain, and I cannot keep popping NSAIDS and Tylenol to keep the pain at bay.
Does anyone have any recommendations as far as doctors to see or any other interventions?
I live 60 miles north of New York City, however, my insurance company does not participate with any of the city's doctors. I can however use Westchester Medical Center and Albany Medical Center.
I have tried physical therapy, massage therapy, chiropractic care and acupuncture.
Someone please help ... I am desperate!
