New Hip Pain After Lumbar Microdiscetomy

I’m 37, 5’3” and 120 lbs. Yep, I’m a pretty big guy. I lead a very active lifestyle and I run competitively. I had a lumbar microdiscetomy L5-S1 on 3/27/2012. Before having the procedure I had been misdiagnosed by several health care professionals. Pulled hamstring, tight periformis…”Here, do these stretches”. X-Rays showed no issues. I’d lived with an intense pain in my right butt down to my calf for about 4 months. When asked then how bad the pain was I’d say about a “6”. (After surgery, I realize the pain was at the least an “8”). Prior to my surgery I was in very poor physical condition. My blood work looked terrible, blood pressure was high, and I felt very anxious and often depressed. Wow, Chronic Pain can really screw you up.

The last physician I saw finally said, "Let’s get an MRI". Bingo! Badly herniated/ruptured disc in L5-S1. He said I was a great candidate for surgery and referred me to a Neurosurgeon. The Neurosurgeon along with her team couldn’t believe how well I was getting around and that I had no muscle loss in my right leg so they operated.

I’m a bit over 2 months out from surgery and the incredible pain I used to get sitting and when going from a sitting position to a standing position has never returned since the surgery. The first two weeks were pretty rough but there is no doubt I’m far better off now than before the surgery. I feel like I’m about 80 to 90 percent.

I was continuing to get better and better until about week 5. I noticed I was getting some hip pain and a bit of nerve pain in my thigh on my right side. Mainly toward the outermost region of my leg. I noticed it then and continue to notice it most when sitting. This pain is different than what I was experiencing before. The pain I had before the surgery was more behind my leg and in my butt and a heck of a lot more painful. I’d give this new pain about a 3.

My current physician thought to give it another month when I told him about it. If it continued or got worse he said he thought it would be a good idea to go to Physical Therapy. He poked and prodded on my hip and believed the problem is still coming from my back. He said it could be scar tissue causing the issue. It's been about 3 weeks since that visit so I’m headed to PT next week.

I admit, I had tried running a bit about a week ago but have stopped. I walk about 2 miles per day and thought running could help. Don’t think it has so I’ve since stopped. By the way, running did not bring the problem on as the new hip/thigh pain was coming on several weeks before.

I have been trying to get back to my old active lifestyle taking it day by day. I've put on about 8 lbs since the surgery and at one time weighed 60 lbs more than I do now. I don't want to go back to being fat which is a big reason for trying to stay active. Maybe I'm pushing myself too much.

Anyone have any ideas what could be causing my new sciatic issues? Ibuprofen, Aspirin, and Muscle Relaxers do nothing for the pain. Cold packs feel nice but a hot bath actually feels the best. Tried a few stretching exercises and seen no improvements. My biggest fear is that this pain will continually get worse.

    My 2-cents of advice may be worth just that.
    I've had 2 MD's at L5-S1. I have hip pain. I found PT helped. I was compensating for so long when I hurt my back, then recovering from my back I was careful to protect it at the cost of making other areas sore.

    The good news? My facial muscles were very tight. I did about 8 weeks of PT with 3 visits to a physiatrist (muscular/skeletal doctor...think body mechanics) and this really helped. I still have hip pain but as long as I keep doing the PT exercises they taught me, I'm able to keep my hip fairly comfortable. It is not uncommon for folks with lower back issues to develop bursitis if they don't properly stretch and make sure they have proper body mechanics in relation to the back.

    I also found that I had to switch up my sleeping approach b/c not only did I have bad hip pain, my Lower Back pain was unbearable in the middle of the night and upon waking (I could barely put pressure on my feet and it would take me15 min to stand up straight). It all came down to relative strength in my back, pelvis, hips. The PT team helped with all that.
