pain medication advice

I’m hoping for some pain medication advice please.

I have severe lower back pain and sciatic pain in my left leg (only mild sciatic pain in my right leg) that started suddenly while working at my computer about 3 months ago.
After 3 days i went to my osteopath (i’ve been seeing her for the past year for long term general back pain that had started to get out of control...1994 broken tailbone, 2000 T12 endplate fracture, 1989-2013 multiple broken ribs and multiple whiplash - adrenalin junkie!) who said L4, L5 and S1 had stopped moving, she freed it up, gave me some stretched to do and said to return immediately if i felt it lock again.
After a further 4 days i couldn’t tolerate the pain anymore and went to my GP who signed me off work for 2 weeks with 30/500 co-codamol, Ibuprofen and Diazipam as my back was so badly in spasm and sent a request to the hospital for me to be seen by the back clinic (we’ve since replaced the Diazipam for Methocarbamol as Diazipam makes me feel like an angel and then fall asleep!).
3 weeks ago i had my assessment at the hospital, they are confident that that all my pain is caused 100% by my muscles, he said all my trigger points from my bra strap to half way down my thighs are in spasm, my muscled are fibrotic indicating that this issue has been building for many years. He said i’m going to need months of PT but currently it’s such a mess he doesn't know what to do with me and that the most important thing to do now is to stop the pain.

So...to cut a long story i have an appointment next week with my GP and would like to request different pain meds (without upsetting her or wasting time in the appointment).
I find the co-codamol restrictive in terms of managing my pain, It can take anything between 15 mins to 60 mins to start working; i haven’t worked out why this is yet, so i don’t take it until i get to work as if it starts working really fast it can make me feel a bit strange and i could be driving at the time, it only seems to provide pain relief for 3 hours max and because it contains paracetamol, i absolutely can’t take any extra.
FYI DOB 1978, weight 55 kg, height 5’5”.

All suggestions/advice welcome.
