Questions about my back pain

So when I about 7 I fell off a horse and got trampled that's when everything seems to start go wrong. After that I started getting back pain after riding or playing softball. I would go to the Dr and they always told me that I pulled a muscle and to rest it and it will be fine. Then later on when I was about 10 they told me I had slight scoliosis and I'd grow out of it. I had kinda given up for a couple years and lived with it not letting my parents see I am still hurting. But 2 weeks ago I had a hard fall it just got worse no matter what I do it's terrible it's mostly in my lower back but is also in my upper back and neck area. I tried icing and heating and even ibuprofen but nothing helps the pain and it's unbearable so I decided to tell my parents. So I went to the Dr the other and the nurse practitioner presses slightly on my back and said I pulled a muscle. Which I really do not believe I have pulled muscles before but never have they lasted this long or hurt this bad I am 17 now I really want actual answers and a Dr that will help me but my doctor won't give me a referral. I didn't even get out of bed the other day I am in so much pain and I left school early today because of it. Does anyone know what I could do?
