Lumbar Artificial Disc Replacement & Cigna

Does anyone know if Cigna covers artificial disc replacement for the lumbar spine? Also if they would cover a 2 level replacement?
Any time I call customer service they won't tell me anything unless I have cpt codes or a request from my doctor submitted.

    Hello...
    My thought is to get that code from your doctor's office and call again.

    If they say approved, or not, I would get their name...and actually, I would call back the next day, surely getting someone else on the phone, and see what they say.
    Insurance companies more unpredictable recently in my experience.
