Problems after disc surgery

I have continued to have problems following the fusion of a disc at L4 and L5.

My surgeon thinks it could be one of two things. The metalwork that is till there or pain from the facet joints.

A SPECT scan has shown that one of the screws is pressing on the iliac bone and he has suggested removal of the metalwork. The scan also showed arthritis in the facet joints.

He also got me to twist "this way and that" which produced pain that he said was coming from the facet joints.

Could the facet joints be fused and the metalwork taken out at the same time and therefore avoid having two ops.
Also, I am about four stone overweight. Is weight losss ever been known to help facet joint problems?

Grateful for any observations.
