Lower right side back pain when moving/tilting head. Need answers, please help

Hi there. I am hoping someone can relate or help me with this problem ive had for 5+ months. I am a severe hypochondriac, always thinking I am dying.

I'm a 20 year old girl, I eat healthy and am in good shape. I do smoke though.

I have this weird pain on the right side of my back that has been going on for 5+ months. Now it ONLY hurts if I "provoke" it by leaning to the right or If I tilt my head down to my chest I get a sharp pain in my lower back and the pain quickly radiates to my right side stomach, above hip and below rib. If I am standing or sitting no pain, If I lay leaned up a bit I get kind of a crampy feel in my side. I dont have pain when sleeping. I have also been really constipated for 5+ months.

I went to the docs about this and she wasnt the least bit concerned. She said it had something to do with a nerve in my back then proceeded to ask me if I hurt my back. (i did do alot of heavy lifting at my old job 5+ months ago when the pain first started) Now the pain has been everyday(only if provoked)
I was tested for UTI, culture was negative.

I dont have a fever, no vomiting etc I feel perfect besides the back thing.

I am so scared because it has not gone away. If anything it may have gotten slightly better, I cant tell. My anxiety is so horrific, I think It may be like spinal cancer or something, idk.

Could it be a pinched nerve/strained muscle? Do they last over 5 months?

Any help would be greatly appreciated. Ty so much.
