Back pain spreads to top of pelvic bone - anyone else?

Hi all,
I've been dealing with regular low back pain for about 20 years now, mainly due to disc compression and herniation in 4,5,s1.  Up until a few months ago, the pain was mostly centralized in the middle of my back.  This time around, I'm having pain out to both sides at the very top of my pelvic bone.  Pain increases greatly when I bend to either side or twist.  PM doc is leaning toward facet syndrome and I have two test blocks scheduled.  I've been working hard in PT but the pain is still ever present.  I'm athletic so this has prevented me from training for months.

While the facet joint theory seems to make sense, the pain at times feels very muscular, like my muscles are tearing away from the iliac crest.  Does anyone else have experience with this sensation?  Thanks!!!
