Thoracic pain at multiple discs

so I'm 34, male, active duty army for the last 14.5 years. I've been having a lot of pain in my t spine and went to my doc about it. The MRI results showed protrusions at t3-4,t4-5,t6-7,t8-9 and a bulge at t10-11. He recommended 3 months of physical therapy the. Go from there. Well 3 months is almost up and my pain is still there if not worse my range of motion has decreased and. NOw my lower back is killing me as well. I'm afraid to go back to him because of how quick the have been putting soldiers in front of a medical evaluation board because I'm so close to retirement, but I need help. I stay active and workout 5-6 days a week plus physical therapy but nothing is helping.
