S1 pain after Microdiscetomy

DC42DC42 Posts: 23
Hi Im a new user, well not really I have had an account for 1 year but can never find out how to use it, and it actually depresses me to read all the non success stories but I guess if you weren't in pain and everything was ok you wouldn't be on here. Well mine is long and complacated but here it is. Had a ruptured disc in Feb. of 09' from heavy lifting at work and a piece of it was pinching the S1 nerve root. Had the microdiscetomy with lam.... sorry about the spelling. Felt good after surgery leg pain was gone only left with numbness. Sent back to heavy manual labor 3 months later with no restrictons. Pain slowly came back in leg but not back. I dealt with the pain until it became unbearible and Im a tuff person. Dr. thought i re-ruptured and ordered MRI but said nothing was wrong. Ordered me off work for 3 more months and I had 3 epidural injections which didnt help at all. Finally after begging long enough he ordered adn emg/ncs which showed left leg radiculpathy. He told me he believed me and my surgery was fbss and I worked through it because he never told me I would feel good just better. He said I had epidural fibrosis on my s1 nerve root and there wasn't much he could do about it, if he removed it more would grow back worse. So he dischared me said Im as good as I will ever be. I still have severe back pain and leg pain all the way to my little toe and side of foot. Pain is unbearable and Dr.s have never gave me any pain meds at all. Finally my own regular Dr. put me on gabapentin which isn't really helping at all, a little mabye. I can not believe they can put me through this without helping me with the pain. Im off work and watching my two very active boys, 2 and 4, and its all i can do to make it through the day. Its not fair two them I can't be the dad I want to be. Im 26 years old and just dont do the things I used to enjoy like, riding 4-wheelers, camping, fishing, hunting, boating etc. Im a very outdoor person, or atleast I was. Anybody have a clue what is wrong with me?

  • Hey there, I don't know what's wrong with you, wish I could offer some help. I'm surprised you were sent back to heavy lifting. Even after my 1st microD, I was told to be extremely careful bending, twisting, lifting for 6 months to a year.

Anyhow, I have a hard time accepting failed back syndrome but I do know it happens. The fact that your first surgery helped initially is encouraging. It's also encouraging that when the pain initially came back it was the leg and not the back. If it's the leg, I suspect there's still some nerve impingement. At least that's what I was told by several specialists.

I am not familiar with gabapentin but from what I read it sounds like Neurontin or Lyrica and acts to 'block' the pain. How long have you been on it? I've heard it can take some time to get into your system before it starts to help.

    Have you tried PT?
Have they done a mylegram or discogram? I forget the name and i know they are controversial but I do know that one of them can help them identify which level is causing the pain when sometimes the MRI can't show it.

The good news is that doctors aren't just offering surgery without a rationale. The disappointing part is that they can't find a source. Any chance scar tissue build up around the S1 nerve is causing the issue?

You can always try another doctor. I'm not sure if you are using an orthospine specialist or a neurosurgeon (both are qualified to work on the back) but it might be worth trying a new view on your case if you can't get your pain under control. At least maybe they can recommend a pain mgmt doctor.

I hope something here helps give you some ideas.

  • Well first you are on work comp so i would request a second opionion asap I di have burning after surgery do to nerve damage doc said it was the nerve healing and it did eventually go away the closest i can explain the pain was kinda like frozen fingers thawing out. If I was you i would see a nerolagist if you saw a spoine secialist and so on. Be pushy to get to an answer you can except. I know kinda how you feel 1 yr after mu sugery i started having knee pain both knees that almost put me out of work wound up being PSA arthritis. Take radiation to mange arhtritis but still have lots of pain and has spread throug my body. Looking at new meds like humira and embril at 3000 dollars a month. The point is push till you get the right answer. I could also tell you my wife had surgery for ovarian cysts, miss diagnosed for a year told she had a bad back, told she had arthrits, told she was depressed and that anti depresant would get rid of her pain. We finnaly after m,any tests and doctors, ultrasounds like 4-5 times, order to do physical therapy. WEll in the end it was ovarian cyststs that almost ruptured her overy. Im sure my spelling is also horible lol. I hope you can get some answers.
