What's this?

I have had lower back pain and sciatica for almost a year now and have a new kind of pain in the last month. All along the pain has been behind me, back, back of legs, buttocks... all in the back. But in the last month the pain is in my outer hips and the front of my thighs down to my knees. It shoots and burns just like the sciatic pain but my understanding is that sciatica only goes down the back... so what is this pain in the front? Is this what SI joint pain is like? It is really bad and keeps me from doing much; even less than before. Walking brings on alot of pain so I'm not even doing much of that now. My doctor said the CT scan showed the herniation was not causing any nerve impingement but that doesn't make any sense to me, especially now. Hopefully the MRI next month will tell what is going on.

  • Pain_is_me,

You might want to look at a Dermatome map as a visual for your new symptoms. I can say this though, I have hernias at the L2/3 and L3/4 levels with stenosis at L3/4. 99% of my issues are upper lumbar back pain and then it radiates to my outer hips, and then wraps around the front of my thighs to the inside of my knees. On really bad days, the top of my calf's sometimes hurts.

Lyrica controls a lot of my nerve pain, but as I found today, some slips past. Today was the electrical zings/burning in the front of my thighs to my knees - ouch. Walking without a cane induces weakness pretty fast, and burning if I try to push it.

Hopefully you're having a fluke, and the MRI will be clear. Please let us know how it goes. Also, have you had electrical studies done on your legs to check nerve status?

    PCTF C4 - T2, Laminectomies C5, C6 & C7. Severe Palsy left arm/hand.
  • before my surgery L5-s1 i had pain down from low back then jumped my but and started just above back of knee all the way down to the tips of my 3 out side toes.
within 2 months it started to go around to the front down to just above my knee. my surgeon say it was probably the nerve was so eiratated that it makes all the nerves around it mad he say wate till the pain gos around to the groin area .well he was right 3 weeks later i went to see him early he say we could try injection . i asked if that would fix it or just put off the knife. he asked if i was ready to do the surgery i say yesterday and pulled out my little pocket knife and told him lets do it right now.
at that point i was taking so much Neroton, and Vicodin. it would have made a horse pass out.in that 3 weeks the pain had gone from like being plugged into a 110 volt plug to a 440 volt plug and my foot and caff was dead feeling besides the electric tingling.
8 weeks after surgery all i had was a numb foot and caff and low back pain and some zapping like a stun gun that would all most put me on the ground. at about 3 months i started with like you out side hip pain. my surgeon say its all normal just nerves waking up. the all the time electric pain was gone thou. but the zapping stun gun was coming back and a deep pain in but and hip like you i though si joint. the pt doc say it was prob from me walking weird and it got me out of aliment.
well almost 10 months later new surgeon new mri slight re-herniated and big tear still like you did not and still dont no why front of hip all most thigh have pain.
hopefully your next mri tells you the story.
    sorry for the rambling and spelling i am just starting lycra again and uping the Flexeril and Percocet 8}
  • I've recently had the same thing, but I've had surgery - have you?

Anyway, my referred pain (it even hurt to breathe sometimes) ended up being mechanical pain from the bones rubbing together below my fusion because my L4/5 can't move and with all my activity, something has to.

So maybe talk to your doc and see if something can be done just for this new symptom. A week of Medrol Pack, back brace and rest helped me tremendously.

I hope you find relief soon.
    Take care,
  • Thanks for the replies. I have not had any surgery yet and just saw the specialist (orthopedic surgeon) for the first time a couple weeks ago.He ordered the MRI as I had not had one yet and said just wait and see what it shows. Ct scan last fall showed herniation at L5/S1 with some disc space narrowing and degenerative 'changes'(whatever that is). My family doc just puts it all off for the specialist to explain so no one can give me answers. I am already taking Lyrica for the nerve pain but its not working very well anymore, I think i need to increase the dose again. I'll try that and see if it improves. Today the pain is wrapping right around my hips to the front and even into the groin area... very painful right where my leg bends in the front to the point where I'm curled up in bed. And lots of those shooting pains too. I am going now to take a percocet and lay on the heating pad - too bad its not bigger, it just doesn't seem to hit all the places I need it but its better than nothing.
No, I have not had any EMG, my doc mentioned it but the surgeon is the only one who can order it and he just didn't seem interested in talking to me at all until I get the MRI.
