Could use some help with a Pain Journal

I was asked by my dr today to start doing a pain journal and an excercise journal, just to document weekly activities
does anyone here know where I could download something to get me started I have been looking for a template but I havent found anything so far.

Some help would be much appreciated, thanks.

  • I did a pain journal and it helped a lot. Basically the headings are Time (every 30 mins) ,What you are doing leave a larger space for this, Pain Level from 01 - 10. It was just typed up in a table in Word. The pain journal really helped me because it made me realise the things I was doing that caused me intense pain like walking the dog which I thought would be good for me. Also driving the car put my pain levels through the roof. I religiously filled it out for 7 days every thirty minutes and could work out that any twisting or pulling motion heightened my pain. It is really worth doing and was one of only good thing I got out of a pain clinic that put me in more pain because it involved exercises where I had to twist and pull. Ironic isn't it.
