Possible Neck Issues? Any Thoughts?!

Sorry this may be kind of lengthy, but I was wondering what could be happening to me. Around September 2012 I started having headaches/migraines. It even sent me to the emergency room (ER) one day. I was at work that day and ended up having to go home because the pain was so extreme. My boss even recommended I should go the ER. They gave me some medication for the migraine and did a CT scan as precautionary; found nothing. I really felt okay since that time until around August 2013 when I would start to get headaches pretty often (as much as 4-5 times per week). I had slight shoulder/neck pain then. By November 2013 the headaches really intensified as did the pain in the neck/shoulders. My family physician ordered a MRI of the brain/head and made a referral to a neurologist. She also sent me to physical therapy for 8 visits. The Physical Therapy did not help at all. The MRI on the brain came back clean (go figure). Well January 2014 rolls around I see the neurologist and I tell him about the headaches and neck/shoulder pain, he said i suffer from migraines and trigger point issues. So he prescribes medication and does trigger point injections in my cervical spine area. The injections only lasted a week and the pain came right back. My headaches aren't nearly as bad (thankfully), but they do come from to time. However; the pain in my neck/shoulder is pretty severe it feels like. It don't seem to be going away. I contacted him again; he wants to do a MRI on my neck (so he ordered) it..I am just awaiting for all that.

What does this sound like it could be? herniated disc? Degeneration? Osteoarthritis? Nerve issues? I am at a lost in though to what it could be. Whatever it is, came on pretty fast in the beginning and has stuck around...

I am only a 21 year old male too and I never had anything like this in my life until now.
