Upper back pain left of spine when moving neck!

I have recently developed a pain in my upper left back which is aggravated when I move my neck. It is aggravated more when i'm looking ahead and tilting my head to the right. (so my right ear touches my shoulder) The pain feels like a severely knotted muscle next to my shoulder blade just under my trap however a knot cannot be felt when massaging it. The pain is not severe but very annoying and noticeable throughout the day. It disappears when I lie flat on the floor. The pain Is not brought on by moving my arms or bending over, just when moving my neck. I also have a stiff neck which feels like it could snap when I tilt my head all the way forward and backward. Very occasionally after waking up in the morning I can feel a very slight pain across my chest muscles, but this disappears after a short while. I've tried some stretches however none have really worked. I was wondering if anyone on hear could partially diagnose my problem just from the information I have provided. I would be very grateful if anyone could offer some tips of exercise to free up the pain. I am a 20 Y/O male, fairly active. I also have a slipped disc at L5 S1, however the pain feels nothing like the pain from that injury.

Thanks !
