help i have fallen

I had L4-S1 fused with titanium screws and rods in july of 2008. I have been mostly without chronic pain for the past year but after a fall a month ago the pain has been intense and it goes down my right leg. My toes and both feet are now mostly numb and i keep losing my balance and falling to the right. Could i have pinched a nerve? I also have ddd and L3-L4 spinal stenosis plus a microdisectomy at C6-C7 with partial fusion and i am a heavy smoker. Please some advice. Thanks
