Bad Pain Right Side Middle of Back

Hi there,
Sorry for the long post.
I have had back pain for a long time and it is getting worse. I just want it gone! It started when I had problems with my left knee, now I have had two knee surgeries and my knee is fine but my back is not.
I recently broke my left wrist and have been doing everything with my right arm which seems to be making it worse, all the lifting etc (I have horses so I'm carting around full wheelbarrow loads of manure, moving hay, carrying 20kg of water, hitching the float onto the truck etc). My back is so sore all the time. I can't find a way to help it!
Movement makes it worse. Driving makes it worse. I have a hot water bottle under my back at night which used to improve it enough I could be normal the next day (until I lifted something heavy again or walked with a limp if my knee was sore) but now its not helping. Ice does not help either. I've told my physio and he shrugs it off and concentrates on the areas I need physio such as my wrist.
My back also means when trimming my miniature horse's feet I can only do one horse before it is too painful to do anything. Sometimes I have been lying in the paddock on my back just concentrating on breathing because the pain is so bad. When trimming feet my back seizes up after one or two feet and it takes me a few minutes before I can walk straight again.
Riding hurts my back as well.
I'm wondering if I should go to a doctor or chiro or get a massage but my mum thinks if I stop doing heavy lifting it will come right but firstly, I can't stop heavy lifting (someone has to muck out after my horses and cart water into their paddock and feed them hay, it is my responsibility to care for them and I intend to do it) and secondly I am 18yrs old and in good health and fitness so I feel that I should be able to lift 20kg and move hay (25kg per bale) and use a full wheelbarrow without worrying about my back.
When lifting I do it properly (bent knees etc).
When my back is sore it is too painful to let someone rub it for me.
I want to live my life with no pain, any ideas appreciated. What should I do? Try to avoid lifting things?
Oh and the pain is on the muscle area on the right middle side of my back. The muscle on that side of my spine is raised higher than on the left side and when walking it bulges out way more than the left side.
I also carry one shoulder slightly higher than the other, not sure if this is related or not?

    On right side I agree with your mom stop lifting the wheel barrels and in my experience no chiro I would see an ortho and maybe you just need to have some Physical
    Therapy ... But you want to rule out herniated disk , compression on nerves, blushing disk ECT... Good luck
