Radio Frequency Ablation- Is anybody's Insurance approving this procedure?


I have had a total of 8 different injections/procedures on my thoracic spine in the last 10 months (5 Medial Branch Blocks, 2 RFA's, and 1 Ephidural).  

I have had a long battle with 3 different insurance companies over the approval of the RFA procedure.  I was originally able to get "Anthem" to approve initially.  They then began denying the procedure claiming it as unproven.  I then moved to "Anthem."  They approved my 2 Medial Branch Blocks but then denied the RFA.  They stated the same thing as United. The procedure is "Unproven."  I was recently denied by a 3rd insurance company for the procedure.

Is anyone having success with the approval of the procedure from a particular insurance company and if so could you share please.  The RFA procedure has been the only helpful procedure for my chronic back pain that I am suffering through daily.
